"I am fully aware of the GMS1 form for new patients that register at a GP practice which is downloadable from the NHS site. However, this is the first time when registering a patient that we have had to fill out a 20 page questionnaire just to get registered. My question is that who is this additional documentation for? What do you get from this questionnaire that you do not already have. I have had to put the same name and address over 4 times on this questionnaire. What information do you get from these that you can not see via NHS records using a simple number (NHS number). I have found this bizarre compared to other GP's who were able to make an appointment within five minutes of me presenting the GMS1 form. I feel that you are trying to take on patients that only you want to take on and are making access to a GP difficult for all. The questionnaire, I intend to contact the NHS direct and ask if these are forms that are allowed to be used in line with their GDPR policy."
