"Both my mother and I were patients at this surgery. When my mother unexpectedly died, the ambulance staff and police were unable to get someone from the surgery to come and certify her death, even though the surgery was quite close by. Likewise, the coroner was unable to get a reply from the surgery and had to contact me twice to ask me to get in touch with them, which I managed to do via the appointments line, as no-one responded to the general number. I needed to do this twice, because despite promising to do so, no-one contacted the coroner after my first call.
While alive, my mother had often had to wait for three weeks for an appointment, despite the serious nature of her condition.
